Okay, here goes. Have some paper in your printer (A "no-printer option" is addressed below).

Fill out this one: Intake Packet

The "no-printer option": If you have Adobe Acrobat Xl installed on your computer, you can open our Intake Packet and follow these instructions to fill out the forms on your computer. Then you can save the file again and email it back to your counselor (A free 30 day trial of Acrobat XL can be downloaded from this page).

How to edit PDF files:

  • Open a file in Acrobat XI.
  • Select Tools > Content Editing.
  • Click the Edit Text & Images tool.
  • Click the on-page element you wish to edit, which automatically activates the tools you'll need.
  • Choose the tool that best fits your task.
